LED Care Pty Ltd’s life stage transition support is intended to assist NDIS participants in navigating significant life transitions with independence and confidence. It is flexible and can be tailored to meet the unique needs of each participant, and our services are provided in a respectful and inclusive manner.

Goal setting and planning: We work closely with individuals with a disability to identify their goals, strengths, and challenges, and develop a personalized plan that sets out the steps needed to achieve those goals. Our team of experienced support workers can help with everything from identifying strengths and interests, to developing new skills, to exploring different pathways and opportunities.

Support with education and training: Education and training can be an important pathway to achieving personal goals and building new skills. Our support workers can help individuals with disability to explore different education and training opportunities, whether that's attending a TAFE or university course, or participating in a vocational training program.
